To fill out the Payment Information Form for a co-op application, follow these steps:
- General Information: Provide the co-op name and the address of the member unit.
- Application Fee: Acknowledge the $201 fee. Choose your payment method:

- Credit Card Payment: Select Visa or MasterCard. Fill in the credit card number, expiry date (mm/yy), cardholder’s name, and signature.
- In-person Payment: Choose to pay by cash, debit card, money order, or certified cheque at an LTB or ServiceOntario location. Provide the name and address of the filing location.
- Mail or Courier: Opt to pay by money order or certified cheque. Address it to the Landlord and Tenant Board, Co-op Processing Office, 47 Sheppard Avenue East Suite 700, Toronto, Ontario M2N 5X5.
