    • Address: Provide the complete address of the recipient, including the street, apartment number (if applicable), city, state, and zip code.
    • Email Address: Enter the recipient’s email address.
  2. Method of Delivery
    • Choose the appropriate method(s) of delivery by checking the corresponding box(es):
      • Electronically to the email address in C: If sending via email, check this box.
      • By email (not through an EFSP): If using regular email and not an Electronic Filing Service Provider, check this box.
      • Using an approved EFSP: If using an EFSP, check this box.
      • Mail or third-party carrier to the address in C, with postage or delivery charge prepaid:
        • Provide the location of the mailbox or third-party carrier from where the documents will be sent, including the city and state.
      • Personal hand delivery at this address: If delivering by hand, provide the delivery address.
      • Mail to the address in C, from a prison or jail: If sending from a correctional facility, provide the name of the prison or jail.
  3. Delivery Date and Time
    • Date: Enter the specific date when the document will be sent (Month, Day, Year).
    • Time: Enter the exact time of sending, including AM or PM.

  1. Recipient Details
    • Name: Enter the full name of the additional recipient.
    • Address: Provide the complete address of the recipient, including the street, apartment number (if applicable), city, state, and zip code.
    • Email Address: Enter the recipient’s email address.

    • Choose the appropriate method(s) of delivery by checking the corresponding box(es):
