   • Read the certification statement carefully. By signing, you certify that the information provided is true and correct, understanding that making a false statement is perjury and has legal penalties.
   • Signature: If you are completing this form on a computer, type your name in the signature field. If you are completing it by hand, sign and print your name.
   • Your Address: Enter your complete address, including street address, city, state, and ZIP code.
   • Telephone and Email: Provide your telephone number and email address. Use an email account that you check regularly to ensure you do not miss important court dates or documents.
