- Housing Specialist Inspection Time: Enter the time when the Housing Specialist will inspect the premises.
- Housing Specialist Inspection Date: Enter the specific date of the inspection.
- Housing Specialist Inspection Year: Enter the year of the inspection.
- Premises Address: Enter the full address of the premises to be inspected.
- Notification Requirement
