For the Notice of Hearing (NoH) packages, indicate if you require any additional documents and specify how you wish to receive these packages, either by email or pick-up at the LTB office. List the addresses of the rental units and application form numbers involved in your bulk filing. If arranging for bulk pick-up, include the date of contact and the set date for pick-up.
