To fill out the N15 form:
- Landlord’s Information: At the top, provide the landlord’s name and the address of the rental unit you’re leaving.
- Tenant’s Information: Write your name(s) as the tenant(s) giving the notice.
- Notice Statement: Clearly state your intention to end the tenancy, specifying the last day of your tenancy in a DD/MM/YYYY format.
- Move-out Declaration: Confirm your plan to move out by the specified date.
- Required Documentation: Attach the required document—a completed LTB form regarding sexual or domestic violence and abuse, or a court-issued restraining order or peace bond from the last 90 days.
- Sign and Date: Provide your signature, the date (DD/MM/YYYY), and, if applicable, the signature of any co-tenants.
- Representative Information: If a representative is acting on your behalf, include their details.
