  1. General Information: Start with your first and last name, distinguishing if you are a landlord or tenant. Enter the address details of the rental unit involved in the application, including the unit/apartment/suite number, street address, city/town, province (defaulted to ON), and postal code. If you have a file number related to the case, include it.
  2. Application/Request Type: Select the type of application or request you are submitting, such as applications regarding the applicability of the Act, rent increases above the guideline, landlord or tenant applications, review requests, or hearing recordings. Each type has a specified fee, so ensure to check the correct box and note the fee amount.
  3. Credit Card Information: Provide your Visa or Mastercard details, including the credit card number and expiry date. Enter the cardholder’s name as it appears on the card and ensure the cardholder signs the form.

Importantly, it advises against uploading the form with credit card information to the Tribunals Ontario Portal for privacy protection, noting that any such uploads will be promptly deleted to safeguard privacy.
